          Instructions for Using Online Forms

1. Search the form you need on Form page.

2. Click the    to open Online Form.

3. Enter your email address. Monarch Registries will use entered email address to contact you if any information is missing on the form.

4. Forms may have one page or a set of pages. Please fill out all the pages.

5. When you fill out a page, you can click the help buttons on the right side of each page to get instructions.

6. After you finish one page, click   on the bottom of the page to check the contents of the form or click   to clear the form.
Note this page is a review page, can't be edited, click   on the bottom of the page to edit the form.

7. If you find any typos, click   on the bottom of the page to edit the form. If the form looks correct, then click  .

8. Some forms allow you to include additional information. You will be given the opportunity to add an additional document at this point. This step is optional. If additional information needs to be attached, put all the additional information it in ONE file and upload the file by clicking  . It might takes a few seconds to upload a large file. If no additional information needs to be attached, leave it blank, click  .

9. At this point, if the form has more than one page, the following page will be displayed for you to fill in. Repeat the same steps as before.

10. After you've finish filling in all the required pages, you will be brought to payment and save page. If you are ready to submit the form, click    and you will be brought to a secure payment page to pay the registration fee. Monarch Registry accepts Visa, MasterCard and American Express.

On the secure payment page, make sure fill in the correct information before you submit payment. Wait till a success page or a failure page appears before you proceed. It might take a while to process payment sometimes. If the payment process fails, that means your bank rejected the payment. Contact your bank for details. Your form is saved in our database. If you can't pay online, please contact us for assistance.

Monarch Registry will email you a payment receipt after payment goes through. Monarch Registry will process your request as soon as possible. Please note services will be processed during our regular business hours only. If you don't receive any emails or calls from Monarch Registry for two days, contact Monarch Registries ASAP. If you are not ready to submit the form, click  . A form number will be assigned to you. Please record your form number as it will allow you to edit your saved form in the future. All the information entered will be saved in the database for 30 days.

11. When you need to update saved form or to pay the registration fee, click  . Put in your email address and form number, then you can access your form. If you forgot your file number, put in your email address, leaving the form number blank. We will email you the form number.
